Advocate Activity Log Keep a running record of your CASA work and send it in at the end of the month.

CASA Contact Log  Use this to complete a more detailed record of a visit or meeting.

Mileage Claim  Fill this out and attach it to your Advocate Log at the end of each month to claim your mileage.

Court Report  Fill this out and send it to the CASA Office at least 3 days before your court hearing.

Annotated Court Report  This will give you more information about how to fill out the Court Report.

Release of Information   This has to be signed by the DHHS case-manager or CFOM to give some one permission to speak to you about the child or family.

JC-caregiver-form     This is the form for foster parents to use to give the court information about how the child is doing.
